Signed-in user session.

**Components:**

- `accessToken`: the bearer access token
- `refreshToken`: the refresh token, or `null`
- `expiresAt`: when the access token expires, in epoch milliseconds
- `project`: the project the user signed in to
- `profile`: the user's FHIR profile
